This course shares information with parents and carers who are supporting a young person with special educational needs and disabilities as they move into adulthood. 

The course will help you to discover the information, tools and support that can support your young person’s journey.  This includes:

  • How to receive advice and resources from local support teams
  • Learn about what key services offer:
    • Supported Living Team
    • Health Transition Team
    • Adult Social Care’s Preparing for Adulthood Team
    • Connexions
    • Learning Disability Nurses

You will also have the opportunity to ask questions, meet with professionals and hear a parent’s real-life journey.
